Former Employee Tax Documents

Beginning with the 2025 tax year, the º£½Ç³Ô¹ÏÍø is no longer required to automatically mail paper copies of Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) to all employees.

You may request a copy of your 2025 Form 1095-C by contacting the Human Resources/Benefits department. We will provide your form within 30 days of receiving your request.

How to Request Your Form

  • Full Name
  • Employee ID Number or last 4 digits of SSN
  • Current Mailing Address or Email Address

Note: While you may need this information for your records, you do not necessarily need Form 1095-C to file your federal income tax return.
